Help, they're having a panic attack! What to do, what not to do, and how to be the anchor in the storm

⚠️
Medical disclaimer The content of this article is for informational and educational purposes only. It does not constitute medical advice and is not a substitute for professional diagnosis, treatment, or therapy. If you or someone you know is experiencing frequent panic attacks or symptoms of an anxiety disorder, please consult a licensed mental health professional or physician. In an emergency, call your local emergency services immediately.

Watching your partner, your child, or your best friend fall apart right next to you is terrifying. That helpless feeling can be paralyzing. But your response matters more than you think.

If someone you love is struggling with panic disorder or panic attacks triggered by obsessive fear of death, you can't be their therapist - but you can be the anchor that pulls them back into reality. Here you'll find practical tips on how to handle a panic attack, and what the difference is between physical panic and a mind spiraling into fear of mortality.

!
The first and most important rule: stay calm.

We know it's hard. But panic is contagious. If they see you panic too, it confirms their catastrophic belief. Your job is to be the calm island in the storm. Speak softly, slowly, and firmly.

The two types - and how to handle each one

Classic panic disorder Physical symptoms - heart attack, suffocation, fainting sensation

The goal is to reduce physical symptoms and redirect attention. They may believe they're having a heart attack, suffocating, or about to faint - their body is receiving real emergency signals.

What to do: Guide their exhale, bring them back to the present, and redirect their attention to the outside world.

Existential panic Thanatophobia - spiral of thoughts, glassy stare

They usually won't let a full panic attack show in front of others, but their body still signals something isn't okay. Your goal: bring them back into the present moment.

Why? Fear of death lives in the future - the present is where safety exists.

Breathing guidance - for physical panic

A panic attack is often maintained by hyperventilation. Don't tell them to "take a deep breath" - guide them to exhale slowly and fully, then inhale, then exhale again with a longer, steadier rhythm.

🌬️
The key is the exhale, not the inhale If you say "take a deep breath," the brain keeps spinning. If you say "breathe out slowly" - the parasympathetic nervous system takes over and the body physically calms down.

The grounding technique - 5-4-3-2-1 method

Shift their brain from internal danger signals to the outside world. Tell them clearly to:

5
👁️ Name 5 things they can see
4
Name 4 things they can touch
3
Name 3 sounds they can hear
2
Name 2 things they can smell
1
Name 1 thing they can taste

Existential panic - what to do and what not to do

❌ What NOT to do
- Don't philosophize. "Come on, that's far away." - This doesn't calm them, because logically they're not wrong. Mortality is real.
- Don't minimize it. "Stop thinking stupid things." - For them, in that moment, it's the most real nightmare they've ever faced.
✅ What to do
- Validate the feeling, not the thought. Show them you understand the fear and that it feels real to them.
- Bring them back to the present. Just like with classic panic - anchor them in real objects, sounds, and concrete right-now details.
- Physical contact. For many, a firm hug is the most effective thing - warmth signals to the brain: you're alive, you're safe. But watch their response - done wrong, it can trigger "what if...?" thoughts.

Banned phrases - never say these!

No matter the type, delete these from your vocabulary. They only pour fuel on the fire:

🚫 "Calm down!" / "Just relax!"
If they could, they would. They don't want to suffer. This only adds frustration and irritation on top of the attack itself.
🚫 "There's nothing wrong with you."
For them, there is. Just because you can't see it or feel it doesn't mean it isn't serious. In that moment, they feel genuine fear of death.
🚫 "Pull yourself together, it's just drama."
Panic isn't a choice. It's a biological and chemical storm in the brain. Drama is about intention - panic is about losing control entirely.

How to help between episodes

The hardest task for loved ones is finding the balance between support and unintentionally maintaining the disorder.

Don't become their "service staff"

If your partner has agoraphobia and you always do everything for them, you help in the short term but reinforce the fear long term. Encourage small attempts - even together with you.

Don't be their therapist

You can listen, but it's not your job to solve their trauma or existential anxiety. That's what professionals are for. They may never open up to you the way they do to a therapist - because shame and relationship fears get in the way.

Take care of yourself too

Living with someone who struggles with panic can be exhausting. You have a right to rest and your own life. If you burn out, you won't be able to help them either.
